Primary Care Physician (MD/DO, FM/IM) – Fort Myers, FL – Full-time OverviewWe are seeking a dedicated and compassionate Primary Care Physician (MD/DO, FM/IM) for a full-time opportunity in Fort Myers, FL. This role is ideal for medical professionals passionate about providing high-quality, patient-centered care within an innovative and supportive healthcare environment. As a Primary Care Physician, you will be an integral part of our mission to improve the health and well-being of our patients through a value-based care approach. Our state-of-the-art neighborhood clinic, opening in May 2025, is designed to create a welcoming atmosphere where patients feel comfortable and respected.
Responsibilities- Deliver comprehensive primary care services in an outpatient setting focused on adult and senior populations, particularly those with multiple chronic conditions such as heart disease, diabetes, and hypertension.
- Work collaboratively within a care team that includes a medical assistant, scheduler, referral specialist, and dedicated leadership support to ensure optimal patient outcomes.
- Manage a panel of up to 650 patients, averaging 10-12 patient visits per day, ensuring continuity and quality of care.
- Utilize a value-based care model to drive positive patient outcomes, including improving patient satisfaction, supporting company values, achieving HEDIS compliance, and reducing hospitalizations and sick days.
- Embrace a Monday through Friday schedule, 8:00 am to 5:00 pm, with a call schedule based on the number of providers in the market (approximately 1:32 weeks).
- Participate in ongoing wellness and community-oriented activities promoted by the clinic, including exercise and art programs for seniors.
- Demonstrate proficiency with office-based procedures, such as suturing, joint injections, skin biopsies, and basic wound care.
- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree required.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM) is required.
- A fellowship in Geriatrics is considered an asset but is not mandatory.
- Active, unrestricted license to practice medicine in the state of Florida.
- Current DEA registration.
- Strong interpersonal and communication skills with a commitment to delivering patient-centered care.
- Proficient in relevant clinical procedures and comfortable using electronic health records and practice management systems.
