Primary Care Physician (MD/DO, FM/IM) – Fort Myers, FL – Full-time
We are seeking a dedicated Primary Care Physician (MD/DO, FM/IM) to join an innovative healthcare provider committed to delivering high-quality, patient-centered primary care in Fort Myers, FL. As a Primary Care Physician, you will play a vital role in an outpatient setting, focusing on a value-based care model that supports continuity, access, and improved wellness for a senior patient population with complex health needs. This is a full-time opportunity, ideal for professionals passionate about making a positive impact on patient outcomes and community well-being.
Responsibilities
- Provide comprehensive primary care to an adult and senior patient population, managing chronic diseases such as heart disease, diabetes, and hypertension.
- Deliver outpatient care in a supportive clinic environment focused on patient satisfaction, quality, and preventive health strategies.
- Work collaboratively within a multidisciplinary care team, including medical assistants, schedulers, referral specialists, and leadership for optimal patient support.
- Maintain a patient panel size of up to 650, averaging 10-12 patients per day to ensure quality and attentive care.
- Participate in a call rotation based on the number of providers in the market (e.g., 1:32 weeks for 32 physicians).
- Engage in continuous quality improvement activities, including meeting HEDIS compliance metrics and contributing to the reduction of hospitalizations.
- Perform basic office procedures as needed, such as suturing, joint injections, skin biopsies, and basic wound care.
- Uphold and promote the company’s core values and commitment to patient-centered care.
Qualifications
- MD or DO degree from an accredited institution.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM).
- Fellowship training in Geriatrics is a plus but not required.
- Current, unrestricted, and active license to practice medicine.
- Valid DEA registration.
- Proficiency in essential office procedures relevant to primary care.
