Primary Care Physician (MD/DO, FM/IM) Job OverviewWe are seeking a dedicated and compassionate Primary Care Physician (MD/DO, FM/IM) to join our innovative healthcare team in Fort Myers, FL. This is a full-time outpatient opportunity offering the chance to make a meaningful difference in the lives of a senior patient population. As a Primary Care Physician, you will play a pivotal role in supporting the health and well-being of our patients, working collaboratively with a supportive care team.
Responsibilities- Provide comprehensive primary care to seniors, managing patients with multiple complex chronic conditions such as heart disease, diabetes, and hypertension.
- Deliver outpatient care in a value-based care model, focusing on patient satisfaction, adherence to clinical guidelines, and improving health outcomes.
- Maintain an average caseload of 10-12 patients per day with a maximum panel size of 650 patients, ensuring personalized and attentive care.
- Collaborate with a dedicated care team that includes a medical assistant, scheduler, referral specialist, and receive full support from leadership.
- Participate in a shared call schedule based on the number of providers in the market, ensuring continuity of care for patients.
- Engage patients in wellness and preventive health activities, including those provided by the clinic such as exercise and art programs, to support holistic health.
- Utilize proficient office procedure skills, including suturing, joint injections, skin biopsies, and basic wound care, as clinically appropriate.
- Support organizational goals by meeting or exceeding key performance indicators such as patient satisfaction, compliance with HEDIS measures, and reduction of hospitalizations.
- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree required.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM).
- Current, unrestricted, and active license to practice medicine in the state of Florida.
- Current DEA registration.
- Fellowship in Geriatrics is an asset but not required.
- Proficient in common office procedures relevant to primary care practice.
