Primary Care Physician (MD/DO, FM/IM) Job Overview
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO, FM/IM) to join our innovative healthcare team in Port Charlotte, FL. This is a full-time opportunity to provide exceptional outpatient primary care services within a value-based care model. Key Responsibilities
- Deliver high-quality, evidence-based primary care to an adult and senior patient population, with a focus on patients managing multiple chronic conditions such as heart disease, diabetes, and hypertension.
- Work within an outpatient clinic setting, ensuring continuity and coordination of care for your panel of patients.
- Manage a patient panel of up to 650 individuals, averaging 10-12 patient visits per day to allow for thorough, personalized care.
- Collaborate with a dedicated care team, including a medical assistant, scheduler, referral specialist, and full support from clinic leadership to optimize patient outcomes and satisfaction.
- Utilize a value-based care approach, focusing on quality metrics such as patient satisfaction, HEDIS compliance, and reduction in hospital sick days.
- Participate in a shared call rotation based on the total number of providers in the local market (e.g., 1:32 weeks if there are 32 physicians).
- Perform minor office procedures such as suturing, joint injections, skin biopsies, and basic wound care as needed.
- Adhere to all clinical protocols, quality standards, and regulatory requirements to ensure patient safety and compliance.
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree from an accredited institution.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM) is required.
- Active, unrestricted medical license to practice medicine in the state of Florida.
- Current and valid DEA registration.
- Fellowship training in Geriatrics is a plus, but not required.
- Proficient in a range of primary care office procedures, including suturing, joint injections, skin biopsies, and basic wound management.
