Primary Care Physician (MD/DO, FM/IM) Position Overview
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO, FM/IM) to join our innovative healthcare team in Port Charlotte, FL. This is a full-time opportunity for a physician interested in providing high-quality, patient-centered outpatient care with a focus on value-based health outcomes. Responsibilities
- Provide comprehensive primary care services to a senior patient population, primarily managing patients with multiple complex conditions such as heart disease, diabetes, and hypertension.
- Deliver outpatient care Monday through Friday from 8 a.m. to 5 p.m., maintaining a manageable daily patient caseload averaging 10-12 patients.
- Utilize a value-based care model to ensure optimal health outcomes and patient satisfaction.
- Coordinate care with a dedicated team including a medical assistant, scheduler, referral specialist, and have full support from leadership to ensure seamless patient experience.
- Adhere to clinical best practices and organizational values, with performance measured by patient satisfaction, HEDIS compliance, and reduction of hospital sick days.
- Participate in a call rotation schedule based on the number of physicians in the market, ensuring work-life balance and shared responsibilities.
- Perform office-based procedures including suturing, joint injections, skin biopsies, and basic wound care as required.
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM) is required.
- Current, unrestricted, and active license to practice medicine in the state of Florida.
- Current DEA registration.
- Proficiency in essential office procedures and electronic medical records.
- Fellowship training in Geriatrics is a plus but not required.
