Responsibilities- Deliver comprehensive outpatient primary care services to a senior patient population, primarily individuals managing multiple chronic conditions such as heart disease, diabetes, and hypertension.
- Engage in a value-based care model, focusing on improving patient outcomes and satisfaction while reducing unnecessary hospital utilization.
- Maintain an average daily patient load of 10-12 with a maximum panel size of 650, ensuring each patient receives attentive and personalized care.
- Work collaboratively with a dedicated care team including a medical assistant, scheduler, referral specialist, and have ongoing support from clinical leadership.
- Participate in activities and programs designed to promote the health and well-being of senior patients, including exercise, art, and wellness initiatives provided at no cost to patients.
- Actively contribute to achieving organizational goals by adhering to best practices in patient satisfaction, compliance with HEDIS measures, and supporting the company’s core values.
- Participate in a shared call schedule, with call frequency determined by the number of providers in the local market (for example, 1 in 32 weeks if 32 physicians are present).
- Perform minor office procedures such as suturing, joint injections, skin biopsies, and basic wound care, as appropriate to the clinical setting.
- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree required.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM).
- Current, unrestricted, and active license to practice medicine in the state of Florida.
- Current DEA registration for prescribing controlled substances.
- Proficiency in minor office procedures relevant to primary care.
- Fellowship training in Geriatrics is considered an asset but is not required.
