Primary Care Physician (MD/DO, FM/IM) – Full-Time
We are seeking an experienced and compassionate Primary Care Physician (MD/DO, FM/IM) to join our innovative healthcare team in Phoenix/Mesa E., AZ. This full-time role offers a rewarding practice setting, working with a collaborative care team and a patient panel that allows for meaningful relationships and impactful healthcare outcomes.
Responsibilities
- Deliver comprehensive primary care services to an outpatient senior population, focusing on individuals with multiple chronic conditions such as heart disease, diabetes, and hypertension.
- Manage a patient panel with an average of 10-12 patients per day, ensuring personalized and attentive care for up to a maximum of 650 patients.
- Work collaboratively with a dedicated care team, including a medical assistant, scheduler, referral specialist, and strong support from leadership to provide seamless patient care.
- Utilize a value-based care model to drive positive patient outcomes and support organizational goals for quality and satisfaction.
- Participate in a rotating call schedule, which is shared equitably among physicians in the Phoenix/Mesa E., AZ market.
- Engage in office-based procedures as appropriate, such as suturing, joint injections, skin biopsies, and basic wound care.
- Contribute to patient wellness by promoting activities and resources designed to improve quality of life for seniors.
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree required.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM) is required.
- Current, unrestricted, and active license to practice medicine in the state of Arizona.
- Current DEA registration.
- Proficiency in common office procedures including suturing, joint injections, skin biopsies, and basic wound care.
- A fellowship in Geriatrics is a plus, but not required.
