Primary Care Physician (MD/DO) Job Overview
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join our innovative healthcare team. Responsibilities
- Deliver high-quality outpatient primary care to a diverse senior patient population, focusing on individuals with multiple chronic conditions such as heart disease, diabetes, and hypertension.
- Utilize a value-based care model to drive improved patient outcomes, emphasizing patient satisfaction, adherence to company values, HEDIS compliance, and reduction of hospital sick days.
- Manage a patient panel size of up to 650, with an average daily volume of 10-12 patients, ensuring continuity of care and personalized attention.
- Collaborate closely with a dedicated care team, including medical assistants, schedulers, referral specialists, and leadership support, to coordinate comprehensive care for each patient.
- Perform office-based procedures as needed, such as suturing, joint injections, skin biopsies, and basic wound care.
- Participate in a call schedule based on the number of providers in the market, ensuring appropriate coverage and work-life balance for all team members.
- Support patient engagement through innovative services such as transportation assistance and wellness-focused activities, including exercise and art, at no additional cost to the patient.
- Document all patient interactions and medical interventions accurately and in a timely manner within the electronic health record system.
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution.
- Board certification or eligibility in Family Medicine (FM) or Internal Medicine (IM) is required.
- Fellowship training in Geriatrics is considered a plus, though not required.
- Current, unrestricted, and active license to practice medicine in the state of Pennsylvania.
- Possess a valid and current DEA registration.
- Proficiency in essential office-based procedures and a commitment to providing comprehensive, patient-centered care.
