Primary Care Physician (MD/DO) Overview
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join our innovative healthcare team in Philadelphia, PA. Responsibilities
- Provide comprehensive primary care to an adult and senior patient population, focusing on the management of multiple chronic conditions such as heart disease, diabetes, and hypertension.
- Deliver outpatient care in a clinic setting with an average of 10-12 patients per day and a panel size of up to 650.
- Work collaboratively with a dedicated care team, including medical assistants, schedulers, referral specialists, and supportive leadership.
- Participate in a value-based care model, emphasizing quality, patient satisfaction, HEDIS compliance, and reducing hospitalizations.
- Perform office-based procedures including suturing, joint injections, skin biopsies, and basic wound care as needed.
- Maintain accurate and timely documentation in accordance with clinical protocols and compliance standards.
- Engage in scheduled shifts from Monday through Friday, 8:00 am to 5:00 pm, with call responsibilities determined by provider count in the region (e.g., 1:32 weeks).
- Support the organization’s commitment to patient-centered care by participating in community wellness initiatives such as exercise and art activities for seniors.
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM).
- Current, unrestricted, active medical license to practice in Pennsylvania.
- Current DEA registration.
- Fellowship training in Geriatrics is a plus but not required.
- Proficiency in office-based procedures and use of PC skills for documentation and patient management.
