Primary Care Physician (MD/DO, FM/IM) - Philadelphia, PA (Full-time) Position Overview
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO, FM/IM) to join our innovative healthcare team in Philadelphia, PA. As a Primary Care Physician, you will play a vital role in delivering comprehensive, patient-centered care within a value-based outpatient model, serving a senior patient population with complex healthcare needs. This full-time opportunity offers a supportive work environment, robust care team resources, and an emphasis on continuity of care and community wellness.
Responsibilities
- Provide high-quality, evidence-based primary care services to a panel of senior patients, averaging 10-12 patients per day in an outpatient setting.
- Manage a patient panel with a maximum size of 650, focusing on individuals with multiple chronic conditions such as heart disease, diabetes, and hypertension.
- Work collaboratively within a multidisciplinary team, including a Medical Assistant, scheduler, referral specialist, and leadership support, to ensure optimal patient outcomes.
- Utilize a value-based care approach to improve patient satisfaction, compliance with HEDIS measures, and reduce hospital utilization and sick days.
- Participate in activities and programs designed to promote the health and wellness of the senior population, including exercise and art activities provided at no cost to patients.
- Document all care and procedures accurately and efficiently in accordance with regulatory requirements and best practices.
- Participate in a shared call schedule, with frequency determined by the number of providers in the market (approximately 1:32 weeks for a group of 32 physicians).
- Perform office-based procedures as appropriate, including suturing, joint injections, skin biopsies, and basic wound care.
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree required.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM) is required.
- Fellowship training in Geriatrics is a plus but not required.
- Current, unrestricted, and active license to practice medicine in Pennsylvania.
- Current DEA registration.
- Proficiency in office-based procedures such as suturing, joint injections, skin biopsies, and basic wound care.
