As a Primary Care Physician (MD/DO, FM/IM), you will play a key role in delivering comprehensive outpatient care, particularly to senior patients managing multiple chronic conditions. This full-time position offers the opportunity to be part of a value-based care model focused on quality outcomes, patient satisfaction, and community wellness.
Responsibilities- Deliver outpatient primary care services to a panel of senior patients, with a typical daily caseload of 10-12 individuals.
- Manage a maximum panel size of 650 patients, focusing on those with complex conditions such as heart disease, diabetes, and hypertension.
- Collaborate with a dedicated care team, including a medical assistant, scheduler, referral specialist, and supportive leadership.
- Utilize a value-based care model, emphasizing patient satisfaction, adherence to HEDIS compliance, and reduction in hospitalizations.
- Participate in a structured call schedule, typically shared across the provider group, to ensure continuity of care without excessive after-hours burden.
- Practice during regular business hours, Monday through Friday, 8:00 am to 5:00 pm, at a new clinic opening May 2025.
- Utilize office-based procedures as needed, such as suturing, joint injections, skin biopsies, and basic wound care.
- Medical degree (MD or DO) from an accredited institution.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM) required.
- Current, unrestricted, and active license to practice medicine in the state of Missouri.
- Current DEA registration.
- Proficiency in common office procedures relevant to outpatient primary care.
- Fellowship training in Geriatrics is a plus but not required.
