Responsibilities
- Provide comprehensive primary care services in an outpatient setting to a panel of senior patients, many with complex chronic conditions such as heart disease, diabetes, and hypertension.
- Deliver high-quality, patient-centered care using a value-based care model that emphasizes prevention, wellness, and improved health outcomes.
- Work collaboratively with a dedicated care team, including a medical assistant, scheduler, referral specialist, and full leadership support, to coordinate patient care.
- Maintain a consistent patient panel size, with an average of 10–12 patients per day and a maximum panel of 650 to ensure personalized attention and optimal care delivery.
- Perform routine office procedures, including suturing, joint injections, skin biopsies, and basic wound care as needed.
- Participate in a shared call schedule based on the number of physicians in the region, ensuring work-life balance for all providers.
- Promote patient engagement by supporting health education, exercise, art, and wellness activities offered at no cost to patients.
- Utilize electronic health records and demonstrate proficient PC skills to document and manage patient care efficiently.
- Adhere to clinical guidelines and participate in quality improvement initiatives, including meeting HEDIS compliance targets and contributing to patient satisfaction goals.
Qualifications
- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree required.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM).
- Current, active, and unrestricted license to practice medicine in the state of Florida.
- Current DEA registration.
- Experience and proficiency in outpatient office procedures, including but not limited to suturing, joint injections, skin biopsies, and basic wound care.
- Fellowship training in Geriatrics is preferred but not required.
- Excellent communication and teamwork skills, with a commitment to delivering compassionate, evidence-based care to a diverse senior patient population.
