Primary Care Physician (MD/DO, FM/IM) Overview
We are seeking a dedicated Primary Care Physician (MD/DO, FM/IM) to join our innovative healthcare team in Phoenix, AZ. This is a full-time opportunity designed for physicians who are passionate about providing exceptional outpatient care to a senior patient population. Key Responsibilities
- Provide comprehensive primary care services to a panel of senior patients, primarily managing chronic and complex conditions such as heart disease, diabetes, and hypertension.
- Deliver outpatient care Monday through Friday, 8 a.m. to 5 p.m., ensuring availability for scheduled appointments and follow-up visits.
- Work collaboratively with a dedicated care team, including medical assistants, schedulers, referral specialists, and support from clinic leadership.
- Maintain a manageable daily patient load (average 10-12 patients per day) and a maximum panel size of 650 patients to ensure personalized and attentive care.
- Participate in a rotating call schedule based on the number of physicians in the Phoenix, AZ market (e.g., 1:32 weeks).
- Contribute to the achievement of key performance indicators, including patient satisfaction, adherence to company values, HEDIS compliance, and the reduction of hospital admissions.
- Utilize proficient procedural skills such as suturing, joint injections, skin biopsies, and basic wound care as needed in the outpatient setting.
Qualifications
- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree required.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM).
- Fellowship training in Geriatrics is a plus but not required.
- Possess a current, unrestricted, and active license to practice medicine in Arizona.
- Current DEA registration is mandatory.
- Demonstrated competency in outpatient clinical procedures relevant to primary care.
