We are seeking a dedicated and compassionate Primary Care Physician (MD/DO, FM/IM) for a full-time opportunity in Greensboro, NC. This role is based in a new, innovative healthcare clinic devoted to improving patient outcomes through best-in-class care and a value-based care model. The Primary Care Physician will be responsible for serving a senior patient population, focusing on continuity of care and comprehensive wellness. Key Responsibilities
- Provide high-quality outpatient primary care to a panel of senior patients, including those with complex chronic conditions such as heart disease, diabetes, and hypertension.
- Practice within a value-based care model, emphasizing patient satisfaction, quality outcomes, and preventive care.
- Manage a patient panel size of up to 650, with an average of 10-12 patients seen per day.
- Work collaboratively with a dedicated care team, including medical assistants, schedulers, referral specialists, and supportive leadership.
- Participate in a shared call schedule based on the number of providers in the market (e.g., 1:32 weeks for a group of 32 physicians).
- Utilize proficient PC skills for office procedures such as suturing, joint injections, skin biopsies, and basic wound care.
- Champion patient-centered care, fostering strong relationships with patients and their families.
- Contribute to a welcoming clinic environment that promotes wellness, including access to activities for seniors to support overall health.
- Ensure compliance with HEDIS measures and support efforts to reduce unnecessary hospital admissions.
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM) (required).
- Fellowship training in Geriatrics is a plus, but not required.
- Current, unrestricted, and active medical license to practice medicine.
- Current DEA registration.
- Proficiency in primary care office procedures, including suturing, joint injections, skin biopsies, and basic wound care.
