Primary Care Physician (MD/DO, FM/IM) – Montgomery, AL – Full-time
We are seeking a dedicated Primary Care Physician (MD/DO, FM/IM) to join our innovative healthcare team in Montgomery, AL. Responsibilities
- Provide comprehensive outpatient primary care to a senior patient population, many with complex chronic conditions including heart disease, diabetes, and hypertension.
- Manage a patient panel with a maximum of 650 individuals, averaging 10-12 patients per day to ensure personalized attention and quality care.
- Collaborate within a value-based care model to improve patient outcomes, satisfaction, and overall wellness.
- Lead and work closely with a dedicated care team, including medical assistants, schedulers, referral specialists, and leadership support.
- Participate in a call schedule based on the number of providers in the market, typically a 1:32 week rotation, ensuring work-life balance.
- Support and engage in initiatives such as exercise, art, and wellness activities provided for seniors, enhancing patient health and community well-being.
- Promote and maintain compliance with HEDIS measures and participate in efforts to reduce hospital admissions and improve care quality.
- Utilize proficient PC skills for office-based procedures, including suturing, joint injections, skin biopsies, and basic wound care.
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree required.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM) required.
- Fellowship training in Geriatrics is a plus but not required.
- Active, unrestricted medical license to practice in the state of Alabama.
- Current and valid DEA registration.
- Strong clinical skills and a commitment to providing quality care in a team-based environment.
