Primary Care Physician (MD/DO, FM/IM) – Full-time – Montgomery, AL
We are seeking a dedicated Primary Care Physician (MD/DO, FM/IM) to join our innovative healthcare team in Montgomery, AL. As a full-time provider, you will play a pivotal role in improving the lives of seniors within a supportive, patient-centered outpatient clinic. Key Responsibilities
- Deliver comprehensive primary care to a senior patient population, managing patients with complex chronic conditions such as heart disease, diabetes, and hypertension.
- Provide care in an outpatient setting, ensuring a personalized approach for each patient.
- Maintain a manageable patient panel, typically seeing 10-12 patients per day with a maximum panel size of 650 individuals.
- Work collaboratively with a dedicated care team, including a medical assistant, scheduler, referral specialist, and supportive leadership staff.
- Contribute to the success of a value-based care model, focusing on patient satisfaction, clinical outcomes, HEDIS compliance, and reducing hospital admissions.
- Participate in a shared call schedule, with frequency determined by the number of providers in the market (e.g., 1:32 weeks if 32 physicians).
- Utilize proficient skills in office-based procedures such as suturing, joint injections, skin biopsies, and basic wound care.
- Support and promote a positive, wellness-focused environment, including patient engagement activities like exercise and art sessions for seniors.
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree required.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM) is required.
- Current, unrestricted, and active medical license to practice in the state of Alabama.
- Current DEA registration.
- Fellowship training in Geriatrics is a plus, but not required.
- Demonstrated proficiency in routine office procedures relevant to primary care.

Employment Type
This is a full-time position, Monday through Friday, 8:00 AM to 5:00 PM, with a collaborative team and a supportive work-life balance.
