Primary Care Physician (MD/DO, FM/IM) – Oklahoma City, OK – Full-time
We are seeking an experienced and compassionate Primary Care Physician (MD/DO, FM/IM) for a full-time opportunity in Oklahoma City, OK. Our innovative healthcare organization is dedicated to enhancing the well-being of our patients by providing exceptional, patient-centered primary care in welcoming neighborhood clinics. As a Primary Care Physician, you will play a vital role in fostering a supportive, wellness-focused environment, particularly for seniors managing complex chronic conditions.
Responsibilities
- Deliver comprehensive outpatient primary care services to a senior patient population, focusing on those with multiple chronic diseases such as heart disease, diabetes, and hypertension.
- Utilize a value-based care model to improve patient health outcomes and reduce hospitalizations.
- Manage a panel of up to 650 patients, averaging 10-12 patient visits per day.
- Collaborate with a dedicated care team including a medical assistant, scheduler, referral specialist, and supportive leadership to provide coordinated, high-quality care.
- Participate in a call schedule based on the number of providers in the market (e.g., 1:32 weeks if 32 physicians are present).
- Practice Monday through Friday, 8:00 am to 5:00 pm.
- Incorporate company values into daily practice and strive to achieve high patient satisfaction, HEDIS compliance, and improved patient outcomes.
- Perform office procedures such as suturing, joint injections, skin biopsies, and basic wound care as required.
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM) – required.
- Fellowship training in Geriatrics is a plus, but not required.
- Current, unrestricted, and active license to practice medicine in the state of Oklahoma.
- Current and valid DEA registration.
- Demonstrated proficiency with office-based procedures such as suturing, joint injections, skin biopsies, and wound care.
