Primary Care Physician (MD/DO, FM/IM) – Sarasota, FL – Full-timeJoin a forward-thinking healthcare provider dedicated to delivering exceptional patient care in Sarasota, FL. As a Primary Care Physician (MD/DO, FM/IM), you will be part of an innovative organization that emphasizes a patient-centered approach, ensuring every individual receives high-quality, accessible medical services. Our organization operates modern, welcoming neighborhood clinics designed to foster a sense of community and continuity of care, especially for senior patients with multiple chronic conditions.
Responsibilities- Provide comprehensive outpatient primary care to an adult and senior patient population, focusing on those with complex health needs such as heart disease, diabetes, and hypertension.
- Work within a value-based care model that prioritizes patient outcomes, satisfaction, and reduced hospitalizations.
- Manage a panel of up to 650 patients, with an average of 10-12 patients seen per day to ensure personalized, high-quality care.
- Collaborate with a dedicated care team, including a medical assistant, scheduler, referral specialist, and supportive leadership, to coordinate and optimize patient care.
- Utilize office-based procedures including suturing, joint injections, skin biopsies, and basic wound care as needed.
- Participate in a call schedule determined by the number of providers in the market (e.g., 1:32 weeks if 32 physicians are present).
- Engage in activities to promote patient wellness, including participation in clinic-based programs such as exercise and art activities for seniors.
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM) required.
- Fellowship training in Geriatrics is a plus but not required.
- Active, unrestricted medical license to practice medicine in Florida.
- Current DEA registration.
- Strong proficiency in primary care office procedures and use of electronic medical records.
