Primary Care Physician (MD/DO, FM/IM) – St. Louis, MO – Full-time
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO, FM/IM) to join our innovative healthcare organization in St. Louis, MO. Responsibilities
- Provide comprehensive outpatient primary care services for a senior population, primarily treating patients with complex conditions such as heart disease, diabetes, and hypertension.
- Manage a patient panel of up to 650 individuals, with an average of 10-12 patients seen per day, ensuring high-quality, personalized care for each patient.
- Work collaboratively with a dedicated care team that includes a medical assistant, scheduler, referral specialist, and full leadership support to deliver coordinated and efficient healthcare services.
- Utilize a value-based care model, focusing on outcomes such as patient satisfaction, adherence to clinical quality measures (including HEDIS compliance), and reduction in hospitalizations.
- Participate in a call schedule based on the number of providers in the market (e.g., 1:32 weeks if 32 physicians are present).
- Maintain standard clinic hours, Monday through Friday, from 8:00 am to 5:00 pm, ensuring availability for patient appointments and care coordination.
- Perform office-based procedures, including suturing, joint injections, skin biopsies, and basic wound care as needed.
Qualifications
- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree from an accredited institution.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM).
- Current, unrestricted, and active license to practice medicine in the state of Missouri.
- Current DEA registration.
- Proficiency in office-based procedures relevant to primary care.
- Fellowship training in Geriatrics is a plus but not required.
