Responsibilities- Provide comprehensive outpatient primary care services for a senior patient population, focusing on those with multiple chronic conditions such as heart disease, diabetes, and hypertension.
- Manage a manageable panel size, with a maximum of 650 patients and an average daily load of 10-12 patients, allowing for thorough, individualized care.
- Work collaboratively with a dedicated care team, including a medical assistant, scheduler, referral specialist, and full leadership support, to optimize patient outcomes and experience.
- Utilize a value-based care model, prioritizing preventive medicine and long-term wellness.
- Participate in quality initiatives, including HEDIS compliance, patient satisfaction programs, and efforts to reduce hospital admissions and sick days.
- Perform office-based procedures such as suturing, joint injections, skin biopsies, and basic wound care.
- Maintain up-to-date documentation and adhere to regulatory and organizational standards.
- Participate in a shared call schedule based on provider volume; for example, a 1:32 week rotation if 32 physicians are in market.
- Attend and contribute to clinic activities designed to promote senior wellness, such as educational and recreational programs.
- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree required.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM) required.
- Current, unrestricted, and active license to practice medicine in Missouri.
- Current DEA registration.
- Proven proficiency in common office-based procedures relevant to primary care.
- Fellowship training in Geriatrics is a plus, but not required.
