Primary Care Physician (MD/DO, FM/IM) – Sarasota, FL – Full-timeOur organization is seeking a dedicated and compassionate Primary Care Physician (MD/DO, FM/IM) to join our team in Sarasota, FL. Our modern, neighborhood-based clinics offer patients a welcoming environment with a focus on continuity of care and individualized attention.
Responsibilities- Provide comprehensive outpatient primary care to a senior patient population, focusing on those with multiple complex medical conditions such as heart disease, diabetes, and hypertension.
- Deliver care within a value-based care model, emphasizing quality outcomes and patient satisfaction.
- Manage an average daily patient load of 10-12, with a maximum panel size of 650 patients.
- Collaborate closely with an assigned care team including a medical assistant, scheduler, referral specialist, and supportive leadership staff.
- Participate in a call schedule based on the number of providers in the market; for example, with 32 physicians, call rotation is 1:32 weeks.
- Engage with patients beyond medical care by supporting wellness activities, including exercise and arts programs offered by the clinic.
- Utilize technology and electronic medical records efficiently for charting and patient communication.
- Ensure compliance with HEDIS quality measures and participate in continuous improvement initiatives.
- Contribute to a positive clinic culture by embracing company values and fostering a respectful, inclusive environment for all patients and staff.
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM).
- Fellowship training in Geriatrics is beneficial but not required.
- Current, active, and unrestricted license to practice medicine in the state of Florida.
- Valid and current DEA registration.
- Proficient in office-based procedures such as suturing, joint injections, skin biopsies, and basic wound care.
