Primary Care Physician (MD/DO, FM/IM) Job OverviewWe are seeking a dedicated and compassionate Primary Care Physician (MD/DO, FM/IM) for a full-time opportunity in Bradenton, FL. This role is ideal for physicians who are passionate about delivering high-quality, patient-centered care within a value-based care model. As a Primary Care Physician, you will play a vital role in improving the health and well-being of seniors in the Bradenton community, working in a supportive outpatient clinic environment.
Responsibilities- Provide comprehensive primary care services to an assigned panel of senior patients, focusing on those with multiple complex chronic conditions such as heart disease, diabetes, and hypertension.
- Deliver care in an outpatient setting, adhering to a Monday through Friday, 8:00 a.m. to 5:00 p.m. schedule.
- Maintain an average daily patient load of 10-12 individuals, with a maximum panel size of 650 patients.
- Collaborate with a dedicated care team, including a medical assistant, scheduler, referral specialist, and leadership support, to ensure continuity of care and seamless patient experiences.
- Participate in a rotating call schedule based on the number of providers in the market.
- Utilize value-based care principles to enhance patient outcomes, including adherence to HEDIS compliance, patient satisfaction, and reduction of hospitalizations.
- Perform common office procedures such as suturing, joint injections, skin biopsies, and basic wound care.
- Engage with patients and families to promote wellness, educate on disease prevention, and foster a welcoming, supportive environment within the clinic.
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree required.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM) required.
- Current, unrestricted, and active license to practice medicine in the state of Florida.
- Current DEA registration.
- Proficient in office-based procedures, including suturing, joint injections, skin biopsies, and basic wound care.
- Fellowship training in Geriatrics is a plus, but not required.
