The available Primary Care Physician position in Winston-Salem, NC, offers a unique chance to be part of a forward-thinking team focused on value-based care, particularly for the senior population living with multiple complex conditions.
Key Responsibilities- Deliver high-quality outpatient primary care to a panel of up to 650 senior patients, focusing on those with chronic conditions such as heart disease, diabetes, and hypertension.
- Provide patient-centered care with an emphasis on continuity, compassion, and building long-term relationships.
- Collaborate closely with a dedicated care team, including medical assistants, schedulers, referral specialists, and supportive leadership, to ensure seamless patient experiences and optimal health outcomes.
- Utilize best practices to promote disease prevention, health maintenance, and improved patient satisfaction in line with value-based care principles.
- Maintain compliance with HEDIS measures and other quality initiatives to drive improved clinical outcomes.
- Engage in regular communication with patients and their families to foster trust and understanding, while empowering patients to participate in their care plans.
- Perform standard office procedures, including suturing, joint injections, skin biopsies, and basic wound care, as needed.
- Participate in a manageable call schedule determined by the number of providers in the area, ensuring work-life balance.
- Contribute to a vibrant clinical atmosphere by supporting activities and programs designed to promote wellness for patients, such as exercise and art classes offered at no cost to participants.
- Medical degree (MD or DO) from an accredited institution.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM).
- Fellowship training in Geriatrics is considered an asset, though not required.
- Active, unrestricted medical license to practice medicine.
- Current DEA registration.
- Proficiency in standard primary care office procedures, including suturing, joint injections, skin biopsies, and wound care.
