As a Primary Care Physician (MD/DO), you will work within a value-based care model that emphasizes quality outcomes, patient satisfaction, and adherence to best clinical practices. You will manage a panel of adult and senior patients, many of whom have complex chronic conditions such as heart disease, diabetes, and hypertension. The average daily patient volume is manageable, allowing you to deliver high-quality, individualized care and build strong patient relationships. You will benefit from a collaborative work environment with strong leadership support, a dedicated care team including medical assistants, schedulers, referral specialists, and access to advanced resources to help you succeed in your role.
Working full-time in this position, you will enjoy a predictable Monday through Friday schedule, with a call rotation that depends on the number of providers in the market. This structure helps ensure a healthy work-life balance while maintaining excellent standards of care for our patients.
Primary Care Physician (MD/DO) - Summary- Deliver comprehensive outpatient primary care services to a panel of adult and senior patients.
- Work collaboratively as part of a multidisciplinary care team dedicated to patient wellness and chronic disease management.
- Engage in a value-based care model focused on patient outcomes and satisfaction.

- Conduct thorough patient assessments, diagnosis, and management of acute and chronic conditions.
- Develop and implement customized care plans for seniors managing multiple complex diseases.
- Utilize evidence-based guidelines and best practices to enhance patient outcomes and quality of care.
- Coordinate care with interdisciplinary team members, including medical assistants, schedulers, and referral specialists.
- Maintain accurate and timely medical records in compliance with regulatory standards.
- Participate in regular quality improvement and compliance activities, including meeting HEDIS measures and reducing hospitalizations.
- Engage patients in preventive care initiatives, health education, and wellness activities provided by the clinic.
- Ensure high levels of patient satisfaction by fostering respectful, compassionate, and effective communication.
- Participate in a rotating call schedule as determined by the provider group size.

- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree required.
- Board Certification or Eligibility in Family Medicine (FM) or Internal Medicine (IM) is required.
- Fellowship training in Geriatrics is a plus, but not required.
- Current, unrestricted, and active license to practice medicine.
- Current DEA registration.
- Proficiency in office procedures such as suturing, joint injections, skin biopsies, and basic wound care.
- Demonstrates a passion for providing high-quality, patient-centered care to senior and adult populations.
