Primary Care Physician (MD/DO) - Summary- Full-time outpatient primary care role focused on adults and seniors
- Value-based care model with an emphasis on patient outcomes and satisfaction

- Provide comprehensive primary care to a panel of up to 650 patients, primarily seniors with complex medical needs
- Deliver care in an outpatient setting, averaging 10-12 patients per day
- Utilize a value-based care model to achieve quality patient outcomes
- Collaborate closely with a care team including medical assistants, schedulers, and referral specialists
- Participate in quality initiatives such as HEDIS compliance, patient satisfaction, and reducing hospitalizations
- Perform office-based procedures such as suturing, joint injections, skin biopsies, and basic wound care
- Engage with wellness programs and community activities provided by the clinic
- Maintain appropriate documentation and regulatory compliance
- Participate in a call schedule determined by the number of providers (e.g., 1:32 weeks)
- Contribute to a culture of respect, empathy, and wellness

- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree required
- Board Certification or Eligibility in Family Medicine (FM) or Internal Medicine (IM) is required
- Fellowship training in Geriatrics is a plus but not required
- Current, unrestricted medical license in the state of practice
- Active DEA registration
- Proficiency in office-based clinical procedures such as suturing, joint injections, and wound care
