Primary Care Physician (MD/DO) - Summary- Full-time opportunity for a board-certified or board-eligible Primary Care Physician (MD/DO)
- Outpatient clinical setting focused on value-based care and patient-centered outcomes
- Manage a senior patient population with multiple chronic conditions
- Work as part of a collaborative, multidisciplinary healthcare team
- Provide comprehensive outpatient primary care to adult and senior patients
- Perform office-based procedures including suturing, joint injections, skin biopsies, and basic wound care
- Develop and implement individualized care plans focusing on the management of chronic and complex diseases
- Adhere to value-based care principles, emphasizing preventive care and quality outcomes
- Participate in quality improvement initiatives and compliance with HEDIS measures
- Collaborate with other healthcare providers and support staff to ensure continuity and coordination of care
- Promote patient education, wellness activities, and community health initiatives
- Utilize electronic health records effectively to document and manage patient information

- Board certified or board eligible in Family Medicine, Internal Medicine, or Geriatrics
- Active and unrestricted medical license
