Primary Care Physician (MD/DO) - SummaryThe Primary Care Physician (MD/DO) will deliver comprehensive outpatient care to a senior patient population, focusing on individuals with multiple chronic conditions such as heart disease, diabetes, and hypertension. This position emphasizes a value-based care approach, prioritizing patient satisfaction, quality outcomes, and reduced hospitalizations. The role is situated in a collaborative clinical environment where physicians are supported in providing personalized, high-touch care while utilizing their clinical expertise in office-based procedures.
Duties & Responsibilities- Provide primary outpatient medical care to senior patients, focusing on those with complex health conditions.
- Utilize a value-based care model to improve patient outcomes and satisfaction.
- Perform office-based procedures such as suturing, joint injections, skin biopsies, and basic wound care as needed.
- Conduct thorough patient assessments, develop individualized care plans, and manage ongoing treatment for chronic diseases.
- Collaborate with multidisciplinary teams to ensure continuity and quality of care.
- Engage patients in wellness initiatives, including exercise and art activities, to promote holistic health.
- Participate in quality improvement programs, including HEDIS compliance and initiatives to reduce hospital admissions.
- Demonstrate adherence to organizational values and contribute to a supportive, respectful clinic culture.
- Document patient interactions comprehensively and accurately in the electronic medical record system.
- Support patient transportation needs by coordinating with available ride services as necessary.

- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree required.

- Proficiency in common office procedures such as suturing, joint injections, skin biopsies, and basic wound care.
- Experience with value-based care models and working with senior patients with multiple chronic conditions is highly desirable.
