Primary Care Physician (MD/DO) - Full-Time

As a Primary Care Physician (MD/DO) in our team, you will be responsible for managing a small patient panel in an outpatient setting, allowing you to provide comprehensive, personalized care to each patient. You will work with a dedicated care team that includes a medical assistant, scheduler, and referral specialist, ensuring you have the support needed to focus on clinical excellence. Primary Care Physician (MD/DO) - Summary
- Full-time, outpatient primary care role focused on value-based care for seniors
- Serve a panel of up to 450 patients, primarily adults with multiple chronic conditions
- Work collaboratively within a physician-led, mission-driven organization
Duties & Responsibilities
- Manage an outpatient panel of approximately 18-22 patients per day
- Develop, implement, and evaluate individualized care plans for senior patients with complex health needs
- Work closely with a dedicated care team including a medical assistant, scheduler, and referral specialist
- Collaborate with in-house consulting specialists such as cardiology and podiatry for comprehensive patient care
- Utilize value-based care strategies to improve patient outcomes and satisfaction
- Conduct patient education on chronic disease management, preventive care, and healthy lifestyle choices
- Document patient encounters and manage administrative responsibilities with scheduled time provided
- Participate in quality improvement initiatives and contribute to a culture of excellence
- Work a Monday through Friday schedule, with clinic hours from 7:30 a.m. to 5:00 p.m. (last patient scheduled at 4:15 p.m.)
- Maintain compliance with all relevant legal, ethical, and professional standards

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution
- Completion of a residency program in Internal Medicine (IM) or Family Medicine (FM)
- Board certified or board eligible in Internal Medicine, Family Medicine, or Geriatric Medicine
- Ability to provide compassionate, patient-centered care to a senior population
- Commitment to value-based care and continuous improvement in patient outcomes
- Excellent communication and teamwork skills
