We are seeking a dedicated Primary Care Physician (MD/DO) to join our innovative healthcare team in Fort Myers, FL. Key Responsibilities
- Deliver high-quality, patient-centered outpatient primary care services, focusing on seniors with multiple chronic conditions such as heart disease, diabetes, and hypertension.
- Maintain a manageable patient panel with an average of 10-12 patients per day and a maximum panel size of 650, ensuring continuity and depth of care.
- Adopt a value-based care approach, contributing to patient satisfaction, adherence to HEDIS compliance, and reduction of hospital admissions and sick days.
- Collaborate with a dedicated care team, including a medical assistant, scheduler, referral specialist, and supportive leadership, to provide holistic patient care.
- Participate in a call schedule determined by the number of providers in the market; for example, with 32 physicians, call rotation is 1:32 weeks.
- Engage patients in wellness activities and community programs aimed at promoting better health independently of direct medical services.
- Utilize proficient clinical skills in office procedures, including suturing, joint injections, skin biopsies, and basic wound care.
Qualifications
- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM).
- Active, unrestricted license to practice medicine in the state of Florida.
- Current DEA registration.
- Fellowship training in Geriatrics is highly valued, though not required.
- Demonstrated proficiency in office-based clinical procedures such as suturing, joint injections, skin biopsies, and wound care.
