Primary Care Physician (MD/DO) Job Overview
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join our innovative healthcare team in Fort Myers, FL. As a Primary Care Physician, you will work within a value-based care model, delivering high-quality, patient-centered outpatient care to a senior population. Responsibilities
- Provide comprehensive primary care services to adults, focusing on the management of chronic and complex medical conditions such as heart disease, diabetes, and hypertension.
- Deliver outpatient care in a clinic setting, averaging 10-12 patients per day with a maximum panel size of 650 patients.
- Collaborate with a dedicated care team, including medical assistants, schedulers, referral specialists, and robust leadership support to ensure optimal patient outcomes.
- Participate in a call rotation schedule based on the number of providers in the market, promoting work-life balance for all team members.
- Use electronic health records and demonstrate proficiency in office procedures such as suturing, joint injections, skin biopsies, and basic wound care.
- Adhere to value-based care principles, focusing on patient satisfaction, quality metrics (such as HEDIS compliance), and reducing unnecessary hospitalizations.
- Engage patients in wellness activities and community-based programs designed to support their overall health and well-being.
- Uphold the organization’s values and contribute to a respectful, inclusive, and patient-focused culture.
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree required.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM) is required.
- Fellowship training in Geriatrics is a plus, but not required.
- Current, unrestricted, and active medical license to practice medicine in the state of Florida.
- Current DEA registration.
- Proficiency in office-based procedures such as suturing, joint injections, skin biopsies, and wound care.
- Strong communication and interpersonal skills, with a commitment to high-quality patient care.
