- Coordinate with multidisciplinary healthcare professionals to ensure integrated and seamless care delivery.
- Maintain accurate and up-to-date electronic medical records in accordance with regulatory and organizational standards.
- Order and interpret diagnostic tests and follow up on results to ensure optimal patient outcomes.
- Provide patient education on preventive care, healthy lifestyle choices, medication management, and chronic disease management.
- Participate in quality improvement initiatives and contribute to the development of best practices within the primary care team.
- Engage in regular professional development activities to stay current with advances in primary care medicine.
Qualifications
- Medical degree (MD or DO) from an accredited institution.
- Completion of a residency program in Family Medicine, Internal Medicine, or a related primary care specialty.
- Board certification or board eligibility in Family Medicine, Internal Medicine, or a comparable primary care discipline.
- Active and unrestricted medical license to practice as a Primary Care Physician (MD/DO) in the state of New Mexico or eligibility to obtain one.
- Strong commitment to delivering high-quality, patient-focused care in a collaborative environment.
- Excellent interpersonal, communication, and organizational skills.
- Proficiency in utilizing electronic health record (EHR) systems for documentation and care coordination.
