Primary Care Physician (MD/DO) Position Overview
We are seeking a dedicated and mission-driven Primary Care Physician (MD/DO) to join our physician-led, primary care organization in New Orleans, LA. Key Responsibilities
- Provide comprehensive outpatient primary care to a panel of senior patients, focusing on those with multiple chronic conditions.
- Manage a small patient panel of up to 450 individuals, allowing for personalized and attentive care.
- Conduct patient visits in a value-based care setting, averaging 18 patients per day.
- Work collaboratively with a dedicated care team, including a medical assistant, scheduler, and referral specialist, to deliver coordinated care.
- Utilize administrative time included within your schedule to ensure efficient patient management and documentation.
- Participate in holistic health initiatives provided by the clinic, such as acupuncture, tai chi, yoga, and nutrition counseling.
- Take part in interdisciplinary collaboration with in-house consulting specialists, including cardiology and podiatry.
- Maintain a work schedule of Monday through Friday, 7:30am to 5:00pm, with the last patient scheduled at 4:15pm. No nights or weekends are required, supporting work-life balance.
- Ensure continuity of care for seniors through proactive health management and preventive care strategies.
- Leverage transportation services provided to patients as needed, ensuring access to care for all members of the community.
Qualifications
- Medical degree (MD or DO) with completion of an accredited residency in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Commitment to value-based care and improving health outcomes for seniors with complex medical needs.
- Strong interpersonal and communication skills to foster positive relationships with patients, families, and care teams.
