Primary Care Physician (MD/DO) Job Description
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join our physician-led, mission-driven primary care organization in New Orleans, LA. This is a full-time opportunity to provide comprehensive outpatient care, focused on serving the senior population with multiple chronic conditions. As a Primary Care Physician, you will play a pivotal role in delivering value-based healthcare and supporting the organization’s mission to provide affordable, high-quality care to those who need it most. The organization is committed to honoring seniors by delivering VIP-level care that drives better health outcomes.
Responsibilities
- Provide comprehensive primary care for a panel of up to 450 patients, focusing on seniors with complex and chronic medical needs.
- Conduct patient consultations, diagnosis, and develop treatment plans for adults, especially those managing 3-5 chronic conditions.
- Work collaboratively with a dedicated care team, including a Medical Assistant, Scheduler, and Referral Specialist, to ensure coordinated, patient-centered care.
- Maintain a Monday through Friday schedule, with clinic hours from 7:30 am to 5:00 pm, and no nights or weekends required, promoting work/life balance.
- Utilize administrative time included within your schedule for charting, care coordination, and team meetings.
- Practice exclusively in an outpatient setting, with no hospital or inpatient responsibilities.
- Participate in value-based care initiatives to enhance patient outcomes and overall clinic performance.
- Leverage access to in-house consulting specialists, including cardiology and podiatry, as well as holistic health services such as acupuncture, Tai Chi, yoga, and nutrition counseling.
- Support patient access to care by coordinating transportation services when needed.
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree.
- Completion of an accredited residency in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board Certification or Board Eligibility in Internal Medicine, Family Medicine, or Geriatric Medicine.
- Commitment to practicing evidence-based, value-driven medicine in an outpatient primary care setting.
- Strong interpersonal, communication, and teamwork skills.
