Primary Care Physician (MD/DO) Overview
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) for a full-time opportunity in Portsmouth, VA. Responsibilities
- Provide comprehensive primary care services to a panel of up to 450 patients, focusing on seniors with multiple chronic conditions.
- Deliver high-quality, patient-centered care in a 100% outpatient setting.
- Collaborate with a dedicated care team, including your own medical assistant, scheduler, and referral specialist, to ensure optimal patient outcomes.
- Manage a daily schedule of approximately 18 patients, with administrative time included to support thorough, value-based care delivery.
- Participate in clinic hours from Monday to Friday, 7:30 am to 5:00 pm, with no nights or weekends required.
- Utilize in-house consulting specialists, including cardiology and podiatry, to provide coordinated care for complex cases.
- Support holistic health initiatives, such as acupuncture, tai chi, yoga, and nutrition programs, as part of an integrated care approach.
- Ensure patients have access to transportation for appointments, supporting continuity of care.
- Practice in a value-based care model focused on improved health outcomes, preventative care, and patient satisfaction.
Qualifications
- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree required.
- Board Certification or Board Eligibility in Family Medicine (FM), Internal Medicine (IM), or Geriatric Medicine.
- Experience or interest in caring for geriatric populations with multiple chronic conditions is preferred.
- Commitment to a patient-focused, team-based care environment.
- Open to candidates requiring visa sponsorship.
