Primary Care Physician (MD/DO) – Cleveland, OH – Full-time About the Opportunity
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join our mission-driven, physician-led organization in Cleveland, OH. Key Responsibilities
- Deliver high-quality, comprehensive primary care services to a panel of senior patients, focusing on the management of chronic conditions such as diabetes, hypertension, and heart disease.
- Maintain a manageable patient panel of up to 450 individuals, allowing for personalized and attentive care.
- Conduct patient visits Monday through Friday during regular clinic hours (7:30 a.m. to 5:00 p.m.), with no night or weekend shifts required, supporting work-life balance.
- Utilize a value-based care model, emphasizing preventative services and holistic health approaches to improve patient outcomes.
- Work collaboratively with a dedicated care team, including a medical assistant, scheduler, and referral specialist, to ensure coordinated and efficient patient care.
- Participate in administrative duties during scheduled time, ensuring thorough and accurate documentation of all patient interactions.
- Collaborate with in-house consulting specialists, including cardiology and podiatry, as well as holistic health professionals offering services such as acupuncture, tai chi, yoga, and nutrition counseling.
- Support the organization’s mission of delivering affordable VIP care to seniors and contribute to continuous improvement initiatives within the clinic.
Qualifications
- Medical degree (MD or DO) with completion of an accredited residency program in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Demonstrated commitment to value-based, patient-centered primary care.
- Strong interpersonal and communication skills, with the ability to collaborate effectively within a multidisciplinary team.
- Dedication to serving seniors and managing complex, chronic conditions in an outpatient setting.
