Primary Care Physician (MD/DO) Job Overview
We are seeking a dedicated Primary Care Physician (MD/DO) to join our mission-driven organization in North Miami, FL. As a physician-led and patient-centered healthcare provider, our organization is committed to transforming primary care for seniors by delivering high-quality, value-based medical services. The Primary Care Physician (MD/DO) will play a crucial role in improving the health and well-being of a senior patient population with complex, chronic conditions. This is a full-time position offering a supportive environment, advanced resources, and a collaborative care team.
Responsibilities
- Provide comprehensive outpatient primary care to a panel of senior patients, focusing on the management of multiple chronic diseases.
- See an average of 18 patients per day to ensure quality, attentive care.
- Deliver care in a value-based model, emphasizing preventive health and long-term wellness outcomes.
- Work closely with a dedicated support team, including a Medical Assistant, Scheduler, and Referral Specialist, to optimize patient care coordination.
- Collaborate with in-house consulting specialists such as cardiology and podiatry to ensure integrated and holistic health services.
- Participate in holistic health programs available onsite, including acupuncture, Tai Chi, yoga, and nutrition counseling, to support patient wellness.
- Maintain a manageable patient panel size, with a maximum of 450 patients, to provide personalized attention.
- Utilize administrative time included within the clinical schedule to complete documentation and follow-up tasks.
- Ensure all care is delivered in compliance with legal, ethical, and organizational standards.
Qualifications
- Medical degree (MD or DO) from an accredited institution.
- Residency training in Internal Medicine (IM) or Family Medicine (FM).
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Commitment to providing patient-centered, value-based care for senior populations.
- Strong communication and collaboration skills to work effectively within a multidisciplinary care team.
Employment Type and Schedule
- Full-time position.
- Clinic hours are Monday through Friday, from 7:30 AM to 5:00 PM, with the last patient scheduled at 4:15 PM.
- No night or weekend shifts required, supporting an excellent work/life balance.
