Primary Care Physician (MD/DO) Job Overview
We are seeking a dedicated Primary Care Physician (MD/DO) to join our innovative healthcare team in Phoenix/Mesa, AZ. As a Primary Care Physician working full-time in our outpatient clinic, you will play a key role in providing comprehensive and compassionate care to a senior patient population with complex health needs. Responsibilities
- Deliver high-quality, evidence-based primary care to adult and senior patients in an outpatient setting.
- Manage a panel of up to 650 patients, focusing on those with multiple chronic conditions such as heart disease, diabetes, and hypertension.
- Average 10-12 patient encounters per day, allowing for thorough assessments and continuity of care.
- Collaborate closely with an assigned care team, including a medical assistant, scheduler, referral specialist, and support from leadership, to optimize patient outcomes.
- Participate in a value-based care model, emphasizing patient satisfaction, clinical quality metrics (including HEDIS compliance), and reducing hospital admissions and sick days.
- Perform office-based procedures such as suturing, joint injections, skin biopsies, and basic wound care as needed.
- Maintain accurate and timely documentation in accordance with regulatory and organizational standards.
- Participate in a rotating call schedule based on the number of providers in the market, ensuring work-life balance and optimal patient coverage.

Qualifications
- MD or DO degree from an accredited institution.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM).
- Fellowship training in Geriatrics is a plus but not required.
- Current, unrestricted, and active medical license to practice medicine.
- Valid DEA registration.
- Proficiency in common office-based procedures and basic computer skills relevant to healthcare settings.
