Primary Care Physician (MD/DO) Overview
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join our innovative healthcare team in Phoenix, AZ. This is a full-time opportunity that allows you to deliver exceptional outpatient care to seniors in a supportive, patient-centered environment. As a Primary Care Physician (MD/DO), you will play a vital role in improving patient outcomes and promoting wellness in the community, with a strong emphasis on continuity of care and value-based medicine.
Responsibilities
- Provide comprehensive outpatient primary care services to a senior patient population, many with complex chronic conditions such as heart disease, diabetes, and hypertension.
- Work collaboratively within a value-based care model to optimize patient health outcomes and reduce unnecessary hospitalizations.
- Manage a patient panel with an average daily census of 10-12 patients and a maximum panel size of 650.
- Lead a personalized care team including a medical assistant, scheduler, referral specialist, and receive full support from clinical leadership.
- Participate in a call schedule determined by the number of providers in the Phoenix, AZ market, ensuring work-life balance for all team members.
- Utilize advanced PC skills in office-based procedures such as suturing, joint injections, skin biopsies, and basic wound care.
- Maintain compliance with HEDIS measures and actively contribute to patient satisfaction and the organization’s mission and values.
- Start date for this position is targeted for May 2025 with the opening of a new center in Phoenix, AZ.
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree required.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM) required.
- Fellowship training in Geriatrics is a plus but not required.
- Active, unrestricted medical license to practice in the state of Arizona.
- Current, valid DEA registration.
- Proficiency in office-based primary care procedures and electronic health record systems.
