Primary Care Physician (MD/DO) About the Role
We are seeking a dedicated Primary Care Physician (MD/DO) for a full-time position at our innovative healthcare facility in Phoenix, AZ. As a Primary Care Physician, you will play a vital role in delivering high-quality outpatient care to a senior patient population with complex health needs. Key Responsibilities
- Provide comprehensive primary care to a panel of senior patients, focusing on those with multiple chronic conditions such as heart disease, diabetes, and hypertension.
- Utilize a value-based care approach to enhance patient outcomes, emphasizing preventive medicine and continuity of care.
- Work collaboratively with a dedicated care team, including a medical assistant, scheduler, referral specialist, and leadership support to ensure coordinated and efficient patient care.
- Maintain an average daily patient load of 10-12, with a maximum panel size of 650 patients.
- Participate in a call schedule determined by the number of physicians in the market, allowing for work-life balance and collaborative coverage (for example, 1:32 weeks if there are 32 providers).
- Document patient encounters accurately and efficiently using electronic health records.
- Perform basic office procedures, including suturing, joint injections, skin biopsies, and basic wound care as needed.
- Support the company mission by promoting wellness and engaging patients in health-focused activities offered at the center.
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree is required.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM) is required.
- Current, unrestricted, and active license to practice medicine.
- Current DEA registration.
- Proficiency in office-based procedures such as suturing, joint injections, skin biopsies, and basic wound care.
- A fellowship in Geriatrics is a plus but not required.
