Primary Care Physician (MD/DO) Overview
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) for an exciting full-time opportunity in Cleveland, OH. Key Responsibilities
- Provide comprehensive outpatient primary care services to a senior patient population, focusing on those with multiple chronic conditions such as heart disease, diabetes, and hypertension.
- Deliver patient-centered, value-based care aimed at improving health outcomes and quality of life.
- Manage an average of 10-12 patients per day with a panel size of up to 650, ensuring high standards of care and continuity.
- Collaborate closely with a dedicated care team, including a medical assistant, scheduler, referral specialist, and leadership support.
- Utilize technology and strong PC skills to perform office procedures such as suturing, joint injections, skin biopsies, and basic wound care.
- Participate in a rotating call schedule, with frequency determined by the number of providers in the market (e.g., 1:32 weeks if 32 physicians).
- Uphold organizational values and contribute to patient satisfaction and compliance with healthcare quality measures.
- Assist in the onboarding and integration of patients to the practice, supporting seamless transitions and continuity of care.
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM).
- Active, unrestricted license to practice medicine in the state of Ohio.
- Current DEA registration.
- Proficiency in relevant office procedures, including suturing, joint injections, skin biopsies, and basic wound care.
- Fellowship training in Geriatrics is a plus but not required.
- Demonstrated commitment to high-quality, patient-centered care and the principles of value-based healthcare delivery.
