Primary Care Physician (MD/DO) Overview
We are seeking a dedicated Primary Care Physician (MD/DO) for a full-time position in Louisville, KY. Our organization is a physician-led, mission-driven primary care provider committed to delivering high-quality, value-based care, with a particular focus on serving the senior population. As part of our team, you will have the opportunity to make a significant impact on patient health outcomes while working in a collaborative and supportive environment.
Key Responsibilities
- Provide comprehensive, outpatient primary care services to a senior patient population, primarily individuals managing multiple chronic conditions.
- Manage a maximum patient panel of 450, allowing for personalized, patient-centered care and strong provider-patient relationships.
- Conduct 18 patient visits per day, ensuring thorough assessment, diagnosis, treatment, and follow-up care in alignment with value-based care principles.
- Collaborate with a dedicated clinical support team, including a medical assistant, scheduler, and referral specialist, to optimize patient care and workflow efficiency.
- Participate in a Monday through Friday clinic schedule, with clinic hours from 7:30 a.m. to 5:00 p.m., and the last patient scheduled at 4:15 p.m.; no nights or weekends required, supporting work-life balance.
- Incorporate administrative time into your daily schedule to complete documentation and coordinate care effectively.
- Utilize in-house consulting specialists, such as cardiology and podiatry, to provide holistic, coordinated care for complex cases.
- Engage in multidisciplinary approaches to health, including access to services such as acupuncture, Tai Chi, yoga, and nutritional counseling.
- Ensure continuity of care by facilitating transportation services for patients who require assistance to attend appointments.
Qualifications
- Medical Degree (MD or DO) from an accredited institution.
- Residency training in Internal Medicine (IM) or Family Medicine (FM) is required.
- Board Certification or Board Eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Demonstrated commitment to patient-centered, value-based care and a passion for serving the senior community.
- Excellent communication, collaboration, and clinical problem-solving skills.
