Primary Care Physician (MD/DO) Job Overview
We are seeking a dedicated Primary Care Physician (MD/DO) for a full-time opportunity in Atlanta, GA. As a member of a physician-led, mission-driven organization, you will play a crucial role in transforming healthcare for seniors through value-based, patient-centered care. Responsibilities
- Deliver comprehensive, high-quality primary care services to a panel of up to 450 patients, primarily seniors managing multiple chronic conditions.
- Manage an average of 18 patients per day in a 100% outpatient setting, ensuring each patient receives individualized attention and evidence-based care.
- Conduct patient assessments, diagnose health conditions, develop treatment plans, and follow up on patient progress.
- Utilize a value-based care model to improve health outcomes, focusing on preventative care and chronic disease management.
- Work closely with a dedicated care team including a medical assistant, scheduler, and referral specialist to optimize workflow and patient satisfaction.
- Participate in interdisciplinary case discussions and collaborate with in-house consulting specialists, such as cardiology and podiatry, as well as holistic services like acupuncture and nutrition.
- Maintain thorough and accurate clinical documentation in compliance with organizational standards and regulatory requirements.
- Dedicate scheduled administrative time to charting, care coordination, and professional development.
- Provide patient education and support regarding disease prevention, medication management, and healthy lifestyle choices.
- Ensure all care is delivered in accordance with ethical, legal, and professional standards.
Qualifications
- Medical degree (MD or DO) from an accredited institution.
- Completion of a residency program in Internal Medicine (IM) or Family Medicine (FM).
- Board Certification or Board Eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Valid and unrestricted medical license to practice in Georgia.
- Demonstrated commitment to providing compassionate, patient-focused care.
- Strong interpersonal, communication, and organizational skills.
- Experience working with older adult populations and managing multiple chronic conditions is preferred.
