Key Responsibilities
- Provide comprehensive outpatient primary care services to a panel of senior patients, focusing on those managing multiple chronic conditions such as diabetes, hypertension, and heart disease.
- Conduct thorough patient assessments, develop individualized care plans, and manage ongoing treatment for a maximum panel of 450 patients.
- Collaborate closely with a dedicated support team, including medical assistants, schedulers, and referral specialists, to ensure seamless patient care and coordination.
- Utilize a value-based care model to optimize patient outcomes, emphasizing preventive care, holistic health, and chronic disease management.
- Participate in the organization’s commitment to work-life balance by maintaining a consistent Monday through Friday schedule, with no night or weekend shifts and administrative time included within standard clinic hours (7:30 a.m. to 5:00 p.m.).
- Engage with in-house consulting specialists, including cardiology and podiatry, to enhance multidisciplinary care approaches for complex cases.
- Promote wellness initiatives such as acupuncture, tai chi, yoga, and nutrition counseling as part of a holistic approach to senior health.
- Ensure compliance with clinical protocols and participate in ongoing quality improvement initiatives.
Required Qualifications
- Medical degree (MD or DO) from an accredited institution.
- Completion of residency training in Internal Medicine (IM) or Family Medicine (FM).
- Board Certification or Board Eligibility in Internal Medicine, Family Medicine, or Geriatric Medicine.
- Demonstrated experience and interest in working with senior populations, particularly individuals managing multiple chronic conditions.
- Excellent communication, clinical, and interpersonal skills, with a commitment to delivering patient-centered care.
