Primary Care Physician (MD/DO) – Richmond, VA – Full-time
Join a physician-led, mission-driven healthcare organization as a Primary Care Physician (MD/DO) in Richmond, VA. As part of a dedicated team committed to transforming healthcare for seniors, you will have the opportunity to provide high-quality, patient-focused care to a panel of older adults with complex health needs. This full-time position emphasizes value-based primary care, allowing clinicians to focus on long-term patient outcomes and continuity of care.
Responsibilities
- Deliver comprehensive primary care to a dedicated patient panel with a focus on seniors, including those with multiple chronic conditions.
- Manage a maximum patient panel of 450 individuals, ensuring personalized attention and quality care for each patient.
- Conduct daily patient visits, averaging approximately 18 patients per day, within a 100% outpatient clinical setting.
- Work collaboratively with a dedicated care and support team, including a medical assistant, scheduler, and referral specialist, to streamline patient care and administrative processes.
- Engage in value-based care initiatives and coordinate with in-house consulting specialists such as cardiology and podiatry to support holistic patient management.
- Participate in in-house holistic health services offered to patients, including acupuncture, tai chi, yoga, and nutrition counseling.
- Maintain thorough documentation and ensure administrative responsibilities are completed during scheduled time blocks.
- Provide clinical leadership and contribute to the continuous improvement of care delivery models focused on seniors’ health.
- No night or weekend shifts required, supporting work-life balance for the care team.

Qualifications
- Medical Degree (MD or DO) from an accredited institution.
- Residency training in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine.
- Demonstrated commitment to value-based care and experience managing patients with chronic diseases is preferred.
- Excellent interpersonal and communication skills, with a passion for providing patient-centered care to seniors.
