Primary Care Physician (MD/DO) Job Overview
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join our multi-site Federally Qualified Health Center (FQHC) team in south central Vermont. This opportunity is ideal for physicians who are passionate about providing comprehensive, family-centered care with integrated behavioral health and care management services. As a Primary Care Physician, you will play a vital role in serving the community, offering preventive health programs such as diabetes education and management, and collaborating with on-site behavioral health and social work professionals.
Key Responsibilities
- Deliver high-quality, patient-centered primary care to individuals and families, addressing a broad spectrum of acute and chronic health conditions.
- Coordinate care with behavioral health providers, social workers, and care managers to support holistic patient wellness.
- Provide preventive care and participate in health education initiatives, including diabetes management programs.
- Maintain a patient schedule averaging 16-22 patients per day, ensuring thorough and timely documentation in the electronic health record.
- Participate in a manageable call schedule, typically 2-3 weeks per year, with no hospital rotations required.
- Engage in collaborative team meetings and quality improvement activities to support the FQHC’s mission and improve patient outcomes.
- Supervise and mentor residents, as 2025 graduates will be considered for this role.
Qualifications
- MD or DO degree from an accredited medical school.
- Board Certified or Board Eligible in Internal Medicine or Family Medicine.
- Active Vermont medical license or eligibility to obtain one.
- Commitment to providing inclusive, patient-centered care to diverse populations.
- Strong teamwork, communication, and organizational skills.
