Primary Care Physician (MD/DO) – Chicago, IL – Full-timeWe are currently seeking a dedicated and patient-focused Primary Care Physician (MD/DO) to join our innovative network of neighborhood primary care centers in Chicago, IL. This full-time opportunity is ideal for a board-certified or board-eligible internal medicine or family medicine physician who is passionate about delivering high-quality, value-based healthcare to local Medicare patient populations.
About the RoleAs a Primary Care Physician (MD/DO) in Chicago, IL, you will play a central role in providing comprehensive patient care as part of a collaborative, multidisciplinary care team. Our care model emphasizes equitable access, preventive medicine, and positive patient outcomes, focusing on quality over quantity. Physicians are supported by a large team including scribes, medical assistants, and social workers, allowing you to focus on delivering exceptional care to each patient. With an average daily patient volume of 13-16 and appointment times designed for both regular and diagnostic visits, you will have the resources and time necessary to address each patient’s unique needs.
Key Responsibilities- Deliver high-quality, value-based primary care services to Medicare patients in Chicago, IL as a full-time Primary Care Physician (MD/DO).
- Work collaboratively with a care team consisting of scribes, medical assistants, and social workers.
- Utilize onsite labs and access to RubiconMD for specialist input to support comprehensive patient care.
- Document and manage patient information using EMR systems including Canopy and Greenway.
- Engage in patient education, preventive care, and chronic disease management.
- Participate in community room activities and support patient wellness initiatives such as exercise, events, yoga, and wellness days.
- Contribute to a practice with an average of 3-5 providers per clinic, including nurse practitioners and other Primary Care Physicians (MD/DO).
- Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) degree from an accredited institution.
- Board certification or eligibility in Internal Medicine or Family Medicine.
- Active, unrestricted, and non-probationary state medical license.
- Bilingual in Spanish is required for effective patient communication.
- Fellowship training in Geriatrics is preferred but not required.
- This position is not open to current residents.
- Value-based care model with a focus on preventive medicine and patient outcomes.
- Patient transportation provided within a five-mile radius of the clinic.
- Supportive and collaborative clinical environment with access to onsite labs and specialist consultations.
