Primary Care Physician (MD/DO) – Gary, IN – Full-time
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join our innovative network of neighborhood primary care centers in Gary, IN. This is a full-time opportunity designed for a board-certified or board-eligible Internal Medicine or Family Medicine physician who is passionate about delivering equitable, value-based healthcare to local Medicare patient populations. Our patient-centered approach and robust care teams enable providers to focus on delivering high-quality care, promoting preventive medicine, and elevating the overall patient experience.
Responsibilities
- Deliver comprehensive primary care to a diverse Medicare population in Gary, IN, emphasizing preventive and value-based care in a full-time capacity.
- Work collaboratively with a multidisciplinary care team, including a scribe, medical assistant, and social worker, to ensure patients receive holistic and coordinated services.
- Manage a daily patient volume averaging 13-16 individuals, allowing ample time for both regular (20 minutes) and full diagnostic or new patient visits (40 minutes).
- Utilize on-site resources such as laboratory services with phlebotomy, and access to RubiconMD for specialist consultation, to enhance patient care and streamline the diagnostic process.
- Engage patients in health-promoting activities through the community room, which hosts exercise classes, events, and wellness initiatives, fostering a supportive healthcare environment.
- Document patient encounters efficiently using Canopy and Greenway electronic medical record (EMR) systems.
- Contribute to a value-based healthcare model designed to improve outcomes, reduce hospital admissions, and maximize patient satisfaction.
- Participate in and support initiatives that improve quality metrics and patient outcomes, which are tied to incentive bonus opportunities.
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathy (DO) graduate from an accredited institution.
- Board certification or eligibility in Internal Medicine or Family Medicine.
- Active, unrestricted, and non-probationary medical license in the state of Indiana.
- Fellowship training in Geriatrics is preferred but not required.
- Current residents are not eligible for this opportunity.
