Primary Care Physician (MD/DO) OverviewPrimary Care Plus is seeking a dedicated Primary Care Physician (MD/DO) to join our expanding team. This full-time position is open to board-certified or board-eligible Internal Medicine and Family Medicine physicians who are committed to delivering comprehensive, patient-centered care. As part of a clinically integrated network, our physicians provide enhanced care to Medicare Advantage, Quality Plan, and Accountable Care Organization (ACO) members. The role is based in Houma, Louisiana, with opportunities to contribute to a collaborative and supportive medical environment.
Responsibilities- Develop, implement, and monitor individualized patient care plans to promote wellness and manage chronic conditions.
- Educate and counsel patients and their families on disease prevention, treatment options, and health maintenance.
- Order, interpret, and evaluate laboratory and diagnostic studies as part of comprehensive patient assessments.
- Prescribe medications and develop treatment regimens tailored to patient needs and current standards of care.
- Refer patients to specialists and coordinate care as clinically indicated for complex or advanced health concerns.
- Maintain thorough and accurate documentation of patient encounters, progress notes, and treatment summaries using an advanced electronic medical record system.
- Participate in patient care rounds, conferences, and interdisciplinary meetings to support coordinated care delivery.
- Collaborate closely with a multidisciplinary team—including APRNs, RN Care Managers, and Quality Improvement staff—to ensure comprehensive and continuous care for all patients.
- Doctor of Medicine (MD), Doctor of Osteopathy (DO), or M.B.B.S degree from an accredited institution.
- Board certification or eligibility in Family Medicine or Internal Medicine.
- Active, unrestricted medical license in the state of Louisiana.
- Current and valid DEA license for prescribing medications.
- Certification in Basic Cardiac Life Support (BCLS).
- Board certification in Internal Medicine or Family Medicine.
- One or more years of clinical experience in a geriatric, adult, or family practice setting.
