Primary Care Physician (MD/DO) Job OverviewPrimary Care Plus is seeking a dedicated Primary Care Physician (MD/DO) to join our expanding team on a full-time basis in Lafayette, Louisiana. As a Primary Care Physician, you will play a crucial role in delivering high-quality healthcare services as part of a clinically integrated network. You will work with Medicare Advantage, Quality Plan, and ACO members, collaborating with a multidisciplinary team to ensure comprehensive patient care. Responsibilities- Develop and implement individualized patient care plans to address the health needs of diverse adult and geriatric patients.
- Provide patient and family education to promote understanding, wellness, and preventive care strategies.
- Order, interpret, and act on laboratory and diagnostic studies to guide patient management and improve outcomes.
- Prescribe appropriate medications, therapies, and treatments in accordance with the latest clinical guidelines and licensure requirements.
- Refer patients for specialty consultations and coordinate specialist care as needed for complex or chronic conditions.
- Maintain thorough and accurate documentation of patient encounters, progress notes, and summaries in the electronic medical record system.
- Participate in patient care rounds, case conferences, and quality improvement initiatives alongside a collaborative healthcare team.
- Work closely with physicians, APRNs, RN Care Managers, and other healthcare professionals to deliver coordinated and patient-focused care.
- Doctor of Medicine (MD), Doctor of Osteopathy (DO), or M.B.B.S degree from an accredited institution.
- Board certification or eligibility in Family Medicine or Internal Medicine is required.
- Active and unrestricted medical license to practice medicine in Louisiana.
- Valid DEA license for prescribing controlled substances.
- Certification in Basic Cardiac Life Support (BCLS).
- Board certification in Internal Medicine or Family Medicine.
- One or more years of clinical experience in a geriatric, adult, or family practice setting.
