Physician Assistant – Primary Care PA-C Job DescriptionFamily Medicine at Greenhill (FMAG) is seeking a dedicated Physician Assistant – Primary Care PA-C to join our team on a full-time basis in Wilmington, Delaware. As a crucial member of our family medicine practice, you will provide comprehensive primary care services to a diverse panel of patients, ranging from newborns to seniors. Our mission is to ensure accessible, high-quality healthcare to all individuals, with a particular focus on supporting vulnerable and underserved populations in Wilmington, Delaware.

We strive to reduce administrative burdens so that our clinicians can focus on diagnosing and treating patients. With the support of a dedicated chronic care management team, behavioral health specialists, and clerical staff, you will have the resources needed to deliver patient-centered care. Our practice offers flexible scheduling options, including early morning, late evening, and Saturday appointments, as well as telemedicine services, to accommodate the needs of our patient community in Wilmington, Delaware.
Responsibilities- Deliver preventive and acute primary care services in an office-based setting to patients of all ages.
- Manage chronic diseases and perform select office-based procedures as appropriate.
- Provide care to patients on your own panel and assist with patients from other clinicians’ panels as needed.
- Participate in flexible scheduling, including some early mornings, evenings, and occasional Saturday mornings.
- Optionally provide in-home primary care visits based on practice needs and your experience.
- Collaborate with a multidisciplinary team that includes physicians, nurse practitioners, nurses, social workers, medical assistants, scribes, clerical staff, and managers.
- Coordinate care with an on-site behavioral health specialist to address mental health and social needs.
- Graduate of an accredited Physician Assistant Program.
- Current licensure to practice as a Physician Assistant in the State of Delaware.
- Active Delaware controlled substance license.
- Current, unencumbered DEA registration.
- Minimum of two years' experience in a primary care or similar clinical setting.
- Pediatric care experience required.
- Experience with in-home patient visits preferred but not required.
- Proficiency with electronic health record (EHR) systems.
- Bilingual abilities are a plus but not required.
- Strong written and interpersonal communication skills.
- Demonstrated ability to work collaboratively within a team to deliver high-quality, value-based care.
